Rights and Responsibilities

Unison Kids Member Rights


Unison Health Plan
of Pennsylvania (CHIP)

 As a CHIP member, you have the following rights:  

  • to choose your child’s own dental, vision, gynecological care and emergency care or mental health and substance abuse services provider. Your child does not need a referral form.
  • to ask for and get information about CHIP and information on how to use CHIP for your child
  • to get good care for your child and to have them treated with respect and due consideration for dignity and privacy
  • to know the names, titles and education backgrounds of all physicians and others helping your child
  • to understand the medical and health needs for your child, what should be done, what choices are available and what risks are involved
  • to be part of any decisions made about your child’s health
  • to be told about what your child should do after a hospital or office visit
  • to say no to treatment;  also, if you don’t want treatment for your child, to take the responsibility and consequences of your decision to say no to treatment for your child
  • to say that you do not want your child to take part in any medical research projects
  • to tell the Member Services Department if you are not happy with Unison Kids
  • to look at all of your child’s medical records in accordance with applicable Federal and State laws; also, to have these records kept private
  • to have an Advance Directive; examples of advance directives:
    -- living will and durable power of attorney
  • to share ideas to make Unison Kids better
  • to file a complaint or grievance regarding any medical or administrative decisions if you disagree, regarding your child. To do this, you need to follow the Complaint and Grievance Procedures of Unison Kids.
  • to give your child’s rights to another person. This would be a person who the law says can make decisions for you about your child’s health care.
  • to be sure the PCP and the staff of Unison Kids know your child’s rights to have these rights with no discrimination of gender, culture, economic, educational, or religious background
  • to receive information on treatment options/alternatives.

 

Unison Kids Member Responsibilities

As a CHIP member, you have the following responsibilities:

  • to understand how CHIP, brought to you by Unison Kids works by reading your child’s member handbook
  • to carry your child’s Unison Kids ID card with you at all times.
  • to make appointments with your child’s PCP for non-emergency treatment each timeyour child needs health care services
  • to get a referral form for your child to make an appointment with a specialist. You need to get this paper from the PCP
  • to be on time for all appointments
  • to tell the PCP’s office or any medical office if you need to change an appointment for your child
  • to respect the rights and property of the PCP, other health care workers, and other patients
  • to do what the PCP tells you regarding your child and to ask questions if you don’t understand
  • to know when your child’s medicine is to be taken, how to take the medicine and to follow the doctor’s instructions
  • to give the right medical information about your child
  • to be sure that the PCP has all your child’s medical records.  This includes all medical records from other doctors
  • to let Unison Kids know if your child is in the hospital or in an emergency room.  Do this in 24 hours or as soon as possible
  • to take full responsibility and consequences of your decision if you refuse (say no) to treatment
  • to consent to the proper use of your health information.
  • If your child is injured by a third party, you may be entitled to receive money from that third party or that third party's liability insurance. Unison has the right to recover and be subrogated to all amounts that it has paid to any health care provider for the treatment of that injury from any responsible third party. You agree to cooperate and agree to direct your attorney, if you have one, to cooperate with Unison in its subrogation to and recovery of those third party liability amounts. You agree to reimburse Unison for the amount to which Unison is entitled up to the amount of settlement, claim, or judgment.
