Unison ID Card Information
New Unison member ID went into effect on Feb. 1, 2010.
The new ID card:

Is easier to read and simpler to use.

Reflects changes to your prescription drug information. See "Prescription coverage changed Feb.1, 2010" for more information.
Once you have your new ID card:

Make sure all your information on the card is correct.

If your PCP or any other information is not correct, please call
Member Services at 1.800.895.2017 (hearing impaired: 711). They will be happy to issue a new card with your correct information. If this happens, you can still use your old card until you receive your new card.

Welcome to Unison Health Plan! As a member of Unison Health Plan, you will receive all of your medically necessary Medicaid covered benefits and extra benefits at no cost to you. We also offer you a large choice of qualified providers and specialists in an area near you. Member Services is available Monday through Friday from 7 a.m. until 7 p.m. to help you with any questions or concerns about your health or the healthcare services you can expect to receive from Unison Health Plan.
A member account is available so you can access your health care information and easily submit questions or comments. Your account will use a secure log-in so that your privacy and personal information is protected. You will need to have your Unison Member ID number to log in. Please call us at 1.800.895.2017 if you need help setting up your account.
- If you receive Healthy Start, Healthy Families, or Aged, Blind or Disabled Medicaid and would like to become a member of Unison Health Plan, please call the Managed Care Enrollment Center at 1.800.605.3040 (TTY 1.800.292.3572). You can also become a Unison Health Plan member by completing an enrollment form at http://www.ohiomcec.com/.
