Unison Advantage (HMO) products are offered by Unison Health Plan, a health plan with a Medicare contract.  Availability of coverage beyond the end of the current year is not guaranteed.    

Eligible individuals may only enroll in a plan during specific times of the year. However, beneficiaries who are eligible for Unison Advantage Plus or Unison Advantage Plus Integrated Care (dual-eligible Special Needs Plans) can enroll at any time.  To obtain more information about enrollment periods, please call our Member Services Department at 1.800.290.4009 (TTY: 711). 

You must be entitled to Medicare Part A and enrolled in Medicare Part B and live in the service area of the plan. However, individuals with End Stage Renal Disease are generally not eligible to enroll in Unison Advantage (HMO) unless they are members of our organization and have been since their dialysis began.  For the Unison Advantage Plus plan and Unison Advantage Plus Integrated Care plan, you must also receive assistance from the state to be eligible.  Please call Member Services at 1.800.290.4009 (TTY: 711) for more information. 

Medicare beneficiaries may enroll in Unison Advantage (HMO) through the Centers for Medicare & Medicaid Services Online Enrollment Center, located at www.medicare.gov. For more information, contact Unison Advantage at 1.800.290.4009 (TTY: 711).   

You must continue to pay your Medicare Part B premium if not otherwise paid for under Medicaid or by another third party.

You must use plan providers except in emergencies, urgent-care situations, or for out-of-area renal dialysis. If you obtain routine care from out-of-network providers, neither Medicare nor Unison Advantage (HMO) will be responsible for the costs.

People with limited incomes may qualify for Extra Help to pay for their prescription drug costs. If eligible, Medicare could pay for seventy-five percent of drug costs, including monthly prescription drug premiums, annual deductibles, and co-insurance. Additionally, those who qualify will not be subject to the coverage gap or a late enrollment penalty. Many people are eligible for these savings and don't even know it.

You may be able to get Extra Help to pay for your prescription drug premiums and costs. To see if you qualify for getting Extra Help, call:

  • 1.800.MEDICARE (1.800.633.4227). TTY users should call 1.877.486.2048, 24 hours a day/7 days a week;
  • The Social Security Office at 1.800.772.1213, between 7 a.m. and 7 p.m., Monday through Friday. TTY users should call 1.800.325.0778; or
  • Your State Medicaid Office.

Unison Advantage (HMO) has contracts with pharmacies (e.g., retail, LTC, home infusion).  Eligible beneficiaries must use Unison Advantage’s (HMO) network pharmacies to access their prescription drug benefit, except under non-routine circumstances when they cannot reasonably use network pharmacies.
