Unison Advantage Plus is a comprehensive health plan that offers the benefits of Original Medicare, plus extra benefits like dental care, eye exams and transportation to your doctor visits.
Unison Advantage Plus is a Special Needs Plan for beneficiaries who receive both Medicare and Medicaid benefits. Eligible beneficiaries can enroll at any time. For more information, call Member Services at 1.800.290.4009 (TTY: 711). You can join Unison Advantage Plus (HMO) if you are entitled to Medicare Part A and enrolled in Medicare Part B and live in the service area. However, individuals with End Stage Renal Disease generally are not eligible to enroll in Unison Advantage Plus (HMO) unless they are members of our organization and have been since their dialysis began. You must also receive assistance from the state to join this plan.
Our 2010 Unison Advantage Plus plan features:
- $0 premiums *
- No referrals needed
- Part D drug coverage with no deductible
- Access to out-of-network doctors and hospitals available under certain conditions
- Preventive dental care
- Eye exams, glasses or contacts
- Hearing aids
- Transportation to doctor visits
(36 one-way or 18 round-trips)
- Low out-of-pocket costs
- $0 copay for up to four routine podiatry visits per year
(Copays and limits may apply)
* You must continue to pay your Part B premium unless otherwise paid for by Medicaid or another third party.

Unison Advantage Ohio Service Area
Unison Advantage products are offered by Unison, a health plan with a Medicare contract. All Medicare beneficiaries who live in the service area may apply.
The Plus plan includes Holmes and Wayne counties.
** You must use network plan providers except in emergencies, urgent-care situations, or for out-of-area renal dialysis. If you obtain routine care from out-of-network providers, neither Medicare nor Unison Advantage will be responsible for the costs. You must use network pharmacies to access your prescription drug benefit, except under non-routine circumstances when you cannot reasonably use network pharmacies. You can obtain a list of Unison Advantage participating providers and pharmacies by contacting the Member Services Department at 1.800.290.4009 (TTY: 711), from 8 a.m. – 8 p.m., seven days a week.
