Unison Advantage is a comprehensive health plan that offers the benefits of Original Medicare, plus extra benefits like denture coverage, transportation to your doctor visits and a fitness center membership to keep you on track for a healthier lifestyle.
Our 2010 Unison Advantage plans feature:

(24 one-way or 12 round-trips)
- Low out-of-pocket costs
- $90 Quarterly Health Products Benefit
(Unison Advantage Plus and Plus Integrated Care plans only)
- $0 copay for up to four routine podiatry visits per year

Unison Advantage offers Unison Advantage Plus and Unison Advantage Plus Integrated Care, Special Needs Plans for beneficiaries who receive both Medicare and Medicaid benefits. Eligible beneficiaries can enroll at any time. You can join Unison Advantage Plus or Unison Advantage Plus Integrated Care (HMO) if you are entitled to Medicare Part A and enrolled in Medicare Part B and live in the service area. However, individuals with End Stage Renal Disease generally are not eligible to enroll in Unison Advantage Plus (HMO) unless they are members of our organization and have been since their dialysis began. You must also receive assistance from the state to join this plan.
Beneficiaries eligible for Unison Advantage Choice or Unison Advantage Basic may enroll in a plan only during specific times of the year. For more information, call Member Services at 1.800.290.4009 (TTY: 711). You can join Unison Advantage Choice or Basic if you are entitled to Medicare Part A and enrolled in Medicare Part B and live in the service area. If you believe you have qualified for extra help and you believe that you are paying an incorrect co-payment amount when you get your prescription at a pharmacy, our Plan has established a process that will allow you to either request assistance in obtaining evidence of your proper co-payment level, or, if you already have the evidence, to provide this evidence to us. You can call Member Services for more information.

Unison Advantage Pennsylvania Service Area
Unison Advantage products are offered by Unison, a health plan with a Medicare contract. All Medicare beneficiaries who live in the service area may apply.
The Choice plan includes the following counties: Adams, Allegheny, Beaver, Berks, Butler, Cumberland, Dauphin, Erie, Lackawanna, Lancaster, Lehigh, Luzerne, Mercer, Northampton, Perry, Somerset, Westmoreland and York.
The Plus plan includes the following counties: Adams, Allegheny, Armstrong, Beaver, Berks, Blair, Butler, Cambria, Clarion, Cumberland, Dauphin, Erie, Fayette, Greene, Indiana, Jefferson, Lackawanna, Lancaster, Lawrence, Lehigh, Luzerne, Mercer, Northampton, Perry, Schuylkill, Somerset, Washington, Westmoreland and York.
The Plus Integrated Care plan includes the following counties: Adams, Allegheny, Armstrong, Beaver, Bedford, Berks, Blair, Butler, Cambria, Clarion, Cumberland, Dauphin, Erie, Fayette, Greene, Indiana, Jefferson, Lackawanna, Lancaster, Lawrence, Lehigh, Luzerne, Mercer, Northampton, Perry, Schuylkill, Somerset, Venango, Warren, Washington, Westmoreland and York.

Unison Advantage (HMO) products are offered by Unison Health Plan, a health plan with a Medicare contract. Availability of coverage beyond the end of the current year is not guaranteed.
Eligible individuals may only enroll in a plan during specific times of the year. However, beneficiaries who are eligible for Unison Advantage Plus or Unison Advantage Plus Integrated Care (dual-eligible Special Needs Plans) can enroll at any time. To obtain more information about enrollment periods, please call our Member Services Department at 1.800.290.4009 (TTY: 711).

Medicare beneficiaries may enroll in Unison Advantage (HMO) through the Centers for Medicare & Medicaid Services Online Enrollment Center, located at www.medicare.gov. For more information, contact Unison Advantage at 1.800.290.4009 (TTY: 711).
You must continue to pay your Medicare Part B premium if not otherwise paid for under Medicaid or by another third party.

You must use plan providers except in emergencies, urgent-care situations, or for out-of-area renal dialysis. If you obtain routine care from out-of-network providers, neither Medicare nor Unison Advantage (HMO) will be responsible for the costs.
People with limited incomes may qualify for Extra Help to pay for their prescription drug costs. If eligible, Medicare could pay for seventy-five percent of drug costs, including monthly prescription drug premiums, annual deductibles, and co-insurance. Additionally, those who qualify will not be subject to the coverage gap or a late enrollment penalty. Many people are eligible for these savings and don't even know it.
You may be able to get Extra Help to pay for your prescription drug premiums and costs. To see if you qualify for getting Extra Help, call:

Unison Advantage (HMO) has contracts with pharmacies (e.g., retail, LTC, home infusion). Eligible beneficiaries must use Unison Advantage’s (HMO) network pharmacies to access their prescription drug benefit, except under non-routine circumstances when they cannot reasonably use network pharmacies.
